Results of Autoclaved Autogenous Segmental bone Graft for Malignant bone and Joint Tumors

Abstract
To investigate the clinical results of autoclaved autogenous segmental bone graft in limb salvage surgery, we studied 14 cases of malignant bone and joint tumors which were treated with neo-adjuvant chemotherapy with cisplatin, adriamicin, methotrexate and ifosfamide followed by limb salvage surgery. Diagnosis were 7 osteogenic sarcomas, 3 chondrosarcomas, 2 solitary plasmacytomas, and rhabdomyosarcoma, malignant lymphoma in each one. The locations were proximal humerus in 7 cases, distal femur in 5 cases, pelvis and elbow in each 1 case. The resected bone segment were autoclaved with 132, 2 atmospheric pressure for 5-10 minutes. The average follow up period wsa 26.3 months(range 12 to 48 months). There were no local recurrence. One died with pulmonary metastasis. One was alive with disease. Twelve were continuous disease free at final follow up. The average bone union time between autoclaved bone segment and host bone was 9.3 months after operation. The union in diaphyseal site was delayed to 2.4 months than that of metaphyseal site. Post-operative complications were developed in 8 cases(57%). There were 5 delayed or nonunion, 1 pathologic fracture with resorption, 1 post-operative infection and 1 shoulder subluxation. Among 5 cases of delayed or non-union, 4 cases were treated with autogenous cancellous bone graft and achieved bone union. The case with pathologic fracture with resorption wsa salvaged with tumor prosthesis.
